ID waiver

The ID Waiver provides home and community-based services to individuals under six (6) years of age who are

DD waiver

The IFDDS Waiver (DD Waiver) provides services to individuals who are 6 years of age and older who have a related condition and do not have a diagnosis of Intellectual Disability who (1) meet the Intermediate Care Facility for the Intellectually Disabled or Persons with related conditions (ICF/ID) level of care criteria (2) are determined to be at imminent risk of ICF/ID placement, and (3) are determined that community-based care services under the DD Waiver are the critical services that enable the individual to remain at home rather than being placed in an ICF/ID.

A related condition is a severe chronic disability, closely related to Intellectual Disability because this condition results in impairment of general intellectual functioning or adaptive behavior, one which manifested before the person reaches age 22, likely to continue indefinitely and results in functional limitation in 3 or more following categories: health status, communication, task learning skills, personal/self care, mobility, behavior, and community living skills. Examples of related conditions may include but not limited to cerebral palsy, muscular dystrophy, shaken baby syndrome, autism, spina bifida and epilepsy.

EDCD waiver

The Elderly or Disabled with Consumer Direction (EDCD) Waiver provides services that help individuals live in their own home or community instead of a nursing home. It is available to individuals 65 years of age and older, and to individuals of any age who have a disability. Individuals who depend on another person for their supports and have medical or nursing needs may be eligible for the EDCD Waiver.

The Technology Assisted Waiver is designed to afford individuals who meet the age appropriate waiver and financial eligibility criteria a choice of home and community based care services rather than reside in an acute care hospital, long stay hospital or specialized care nursing facility.
